ALL ACTIVITIES WAIVER
By registering for this activity I acknowledge and understand the following: *CHILDREN MUST BE AT LEAST 5 YEARS OF AGE BY THE START DATE OF THE CAMP OR CLASS FOR WHICH THEY ARE REGISTERING! THE BURDEN OF PROOF IS ACCEPTED BY PARENT/GUARDIAN, WHO IS PREPARED TO PROVIDE PROOF OF AGE UPON REQUEST.
AUTHORIZATION FOR EMERGENCY MEDICAL CARE I am aware of my child's general condition and affirm that he/she is fit to participate in any activities required for participation in this program. I will fully disclose any relevant medical information to Huck Finn School, LLC and its staff, and will engage in all prescribed activities except for those noted by me and/or by my examining physician. In the event my child is rendered unable to communicate by an emergency or accident, I authorize and request such medical and surgical services as may be necessary, and further agree to accept financial responsibility for the same. Should we be unable to contact the named individuals, secondary contacts or a family physician, permission is hereby granted for medical care to be provided as required. The registering parent/guardian agrees to be responsible for any fees that may be incurred.

ANYONE WITH SEVERE ALLERGIES TO FOOD, PLANTS OR INSECTS MUST INFORM HUCK FINN SCHOOL STAFF AND BRING THEIR OWN MEDICATION, AND BE ABLE TO ADMINISTER IT!
HOLD HARMLESS AGREEMENT By registering for this activity I hereby clearly and unequivocally express my intent to release, discharge, and hold harmless Huck Finn School, LLC, its owners, members, managers, officers, directors, employees, partners, joint venturers, volunteers, rental property owners, successors, and assigns, the town of Sullivan’s Island, Sullivan's Island Baptist Church, the Bronco family, the town of Awendaw, Camp Sewee and its assigns, KJWE LLC., Nature Adventure Outfitters, the town of McClellanville and the Lutheran Church from any and all claims or causes of action for compensation for physical injury or property damage arising out of or in any way connected with my participation in Huck Finn School, LLC activities, even if such physical injury or property damage is caused by the members, managers, officers, directors, employees, partners, joint venturers, volunteers, successors, and assigns. I hereby waive any such claims or causes of action for compensation for physical injury, accident, loss or damage of personal property caused either by negligence or risks inherent in the activity.
By registering I am representing that I understand, comprehend, and appreciate that certain risks and dangers exist in activities that may take place during classes, camps, trips or programs. These risks may include, but are not limited to loss or damage of personal property, injury or fatality due to tripping over roots, falling from heights, animal bites or stings, drowning, and allergic reactions to foods or insects, use of tools, exposure to temperature extremes or inclement weather, sunburn, and vehicle accidents while traveling to and from the activity site.
I acknowledge and assume all risks in connection with this activity, and I hold Huck Finn School, LLC, the Sullivan's Island Baptist Church, the town of Sullivan’s Island, the Bronco family and the town of Awendaw, Camp Sewee and its agents, KJWE LLC., Nature Adventure Outfitters, the Town of McClellanville and The Lutheran Church harmless from any and all liability, actions, claims and damage of every kind. I am aware that my child is participating at his/her own risk as it relates to the Covid 19 Virus and do not hold Huck Finn School, LLC or its assigns responsible for any illness during or following their participation. Furthermore, I hereby grant permission to all of the foregoing to use my photographs, motion pictures, recordings, or any other record of this event for any legitimate purpose.
By registering for this activity, I hereby certify that I am the parent or legal guardian of the participant, and I have read the foregoing agreement and agree to its terms and conditions on behalf of myself, all other parents or guardians of my child, and my minor children.
